Applying for an operating license for a pharmacy

• German license to practice as a pharmacist

• the equipment required to operate a pharmacy

• Curriculum vitae
• Notarized license to practice medicine
• Medical certificate
• Proof of professional activities
• Rental and purchase contracts
• Various written declarations
• Official certificate of good conduct document type 0 (zero)

A pharmacy may only be opened after the competent authority has certified that the pharmacy meets the legal requirements (acceptance). The license expires, among other things, if it has not been used within one year.

The license to operate a pharmacy is issued within two months of submission of the complete application documents at the latest.

An appeal may be lodged against the decision.

• Various forms and declarations that can be requested from the responsible office

Note: Official certificate of good conduct document type 0 (zero) must be applied for.

The permit expires, among other things

• by death,
• by renunciation,
• by withdrawal or revocation of the license to practice as a pharmacist,
• if no use has been made of the license for one year (the competent authority may extend the period if there is good cause).
