You must submit the following evidence together with the notification:

Proof that you are responsible for the safe performance of the activity
- have appointed the necessary number of radiation protection officers and
- have granted them the necessary authorizations.
- Proof that each radiation protection officer has the necessary expertise in radiation protection or

If no person responsible for radiation protection is required, proof that
- the person required to report,
- their legal representative or
- a person authorized by law, articles of association or partnership agreement to represent or manage the company
has the necessary expertise in radiation protection.
- Proof that the persons otherwise involved in the activity have the necessary knowledge and skills with regard to radiation hazards and protective measures.
- Plausible explanation of the intended procedure for determining the individual body dose in accordance with the regulations of radiation protection law.
In the case of a separate notification, you must also submit the following evidence:
- Demonstration that the increased exposure is justified,
- Proof that the increased exposure has been discussed with the personnel to be deployed and the authorized physician,
- Proof that the personnel to be deployed has been informed about the expected doses, the risks associated with the increased exposure and the precautionary measures to be taken,
- consent of the personnel to be deployed to the increased exposure.
